At Disability Macarthur Family support and care, we are committed to client satisfaction and providing a quality service that is meeting your needs.
We are pleased you have continued to use Disability Macarthur for your respite requirements. The effectiveness of the organization rests not only on your satisfaction in our service, but also in your ability to get your questions answered promptly and efficiently.
To make sure we’re meeting your needs, we have enclosed a client service survey. We invite you to share your opinions on how well our staff were able to help you with your problems and questions when delivering the service.
Please return the completed survey in the enclosed postage-paid envelope.
All survey that are returned will go into the draw for a family pass to the movies. To be eligible for the draw you will need to provide your name and contact phone number at the end of the survey, and return by the 17th October 2014.
We will be posting this survey on our website in the coming days if you would prefer to lodge it electronically at www.disabilitymacarthur.org.au.
